Telemed Options

shutterstock_1683359032

Holy Savior Hospice and Holy Savior Palliative Care provide services through telecommunication technology; including telehealth visits and remote patient monitoring. This is available for patients and families that want it allows clinical staff to answer questions, discuss pain management, assist with medicine dosage information, and more while limiting face-to-face contact; which is increasingly important during COVID-19.

How Does Telehealth Work?
Telehealth is a visit with your doctor, nurse practitioner, or nurse over a smartphone, table or laptop.

Patients are now able to receive a video call from their doctor, nurse practitioner, or nurse that gives them the ability to see them, discuss their clinical condition and how things are going, just like if they were visiting the patient in their home

Remote Patient Monitoring

We offer 24/7 monitoring of patient's vital signs for early identification of potentially complex changes in medical condition.

For Providers

We will work with you and your patients to provide the highest quality of care.

  • We are able to provide care, medications, medical supplies and durable medical equipment, anywhere the patient calls home, whether that’s assisted living, a nursing home or a private residence.
  • We provide a highly experienced interdisciplinary team comprising of physician, registered nurse, certified nursing aide, social worker and chaplain.
  • We provide each patient with a tailored care plan that will prioritize their comfort and overall quality of life.
  • We are family owned and operated.
  • We offer 24 hour access to clinical, emotional and spiritual support.
  • We provide family and loved ones with ongoing education, guidance and support.
  • No long term commitment, freedom to discontinue care at anytime.
  • We offer daily Hospice Aide Services based on a patient's personalized plan of care, in hopes of reducing caregiver burnout allowing for quality time spent with their loved one.

There is a common misconception that choosing hospice means giving up hope. The facts about hospice, however, prove otherwise. Given the time, hospice actually relieves suffering, promotes dignity, and facilitates closure for patients and families. hospice patients are often able to live out their remaining days, weeks, and months more comfortably—and in many cases, for longer periods—than they would if they had continued seeking curative treatment until the point of death.
